In advance of the agreement, Matria completed the development of TRAX, a new proprietary patient management and clinical record system, to support the Medical Mutual agreement, and other disease management contracts. Frank Powers, President of Population Health Management for Matria, stated: "Our agreement with Medical Mutual is validation of Matria's disease management diversification. We now serve Medical Mutual members with programmes for asthma, chronic obstructive pulmonary disease (COPD), high-risk pregnancy and diabetes."
Matria care managers will call all Medical Mutual members who have been diagnosed with diabetes. After agreeing to take part in the programme, each member will be evaluated so that a patient-specific course of treatment can be determined. Interactive health profiles will be completed at the time of enrollment. Each participating member ordering through Matria, will also receive, free of charge, supply requirements, such as glucometers to measure blood sugar and test strips.
At regular intervals, participants receive proactive care calls during which each participant's compliance with the provider recommendations and adherence to the diabetes self-care programme are monitored. Members will also receive diabetes patient education and have telephone access to a Matria care management team which includes certified case managers, registered nurses and certified diabetes educators.

Once identified, Matria case managers can utilise the system to manage clinical interventions. TRAX is a Web-based process automation platform that connects patients, care co-ordinators, health plans and providers through a virtual private network. Individual plan members and entire health plan populations can be managed through the TRAX system."
